1. What are your professional credentials?
2. Can you provide me with references?
3. Do you have a license in your profession?
4. Are you a member of the National Association of Professional Geriatric Care Managers?
5. Who provides the services and who is their back-up?
6. How long have you been in the business of providing care management services?
7. How do you handle emergencies?
8. Does your company also provide home care services?
9. How do you communicate information to family members?
10. What are your fees? (These should be provided to the consumer/responsible party in writing prior to services starting.)
11. What are your payment methods?
